SEND Family Services

The Family Services Teams work to support children with Special Educational Needs and Disabilities (SEND), their families and the education settings they attend.

The teams:

  • Ensure that assessments, including Education, Health and Care Needs Assessments, are accurate and delivered within statutory timescales.
  • Issue and review Education, Health and Care Plans (EHCPs).
  • Monitor the progress of children and young people with SEND in achieving outcomes from their Education, Health and Care Plans.
  • Support children and young people with Education, Health and Care Plans who are at risk of being, or who have been permanently excluded.

Education, Health and Care (EHC) Portal

In Suffolk, we use an online portal for Education, Health and Care Needs Assessment (EHCNA) requests, which you can access via the following link: 

If you have previously submitted an Early Help Assessment (EHA), formerly known as a Common Assessment Framework (CAF), or safeguarding referral to us, you will already have an account. If not, you will be asked to create an account the first time you complete a request. You will find guidance on how to complete the new portal form using the link above.
