The survey is hosted by the Healthwatch Suffolk service on behalf of the Local Area Partnership for SEND in Suffolk. It is made up of organisations supporting young people like Suffolk County Council and the NHS.
How to take part
Young people aged 13 – 25 with SEND can share their views, as well as local parents, carers, and guardians.

- Parents, carers, and guardians can take part in an online survey at: www.smartsurvey.co.uk/s/PfAParentFeedback/
- There is also an opportunity for young people aged 21-25 to take part in conversations with the Healthwatch team. Please sign-up at: https://healthwatchsuffolk.co.uk/pfasignup/

British Sign Language support to take part is also available to young people and parents by finding Healthwatch Suffolk on the Convo Now UK app. You can find this service on Google Play and the Apple App Store.
Please support young people to take part if you can
The survey aims to be accessible to as many young people as possible. However, it is recognised that some young people may need help from a trusted adult to take part.
If you can, please share the survey with a young person you support or care for and help them to complete it. It is possible to pause the survey and return to complete it later if you need more time.

What does the Local Area Partnership want to know?
The survey is about the support available to young people with SEND in Suffolk to help them prepare for their adulthood. It will support organisations and services like schools, colleges, Suffolk County Council, and the NHS to work on a plan for how young people can be supported locally now, and into the future.
The surveys will ask about things like:
- The choices young people have about their education and getting ready for the working world.
- What is needed to help young people take care of their own health.
- Learning life skills like managing money, cooking and cleaning.
- Feeling happy to get around or do things in life without support.
- Finding support and friends in local communities.
